Forms
Build forms on your landing pages to capture customer details, reviews, and purchase info.
Forms are how Swapt collects first-party data. You add a Form block to a landing page, and every submission creates or updates a customer and moves the customer on through your flow.
Add a form
In the page editor, drag a Form block onto your page. It starts with First Name, Last Name, Email, and Phone Number fields.
In the block's settings, click Edit Form Builder. The Form Builder opens with three panels: Form Structure, Field Settings, and Live Preview.
Click Add field to add fields, or Add step to split the form across several screens.
Click Design to choose what happens after someone submits (see below), then save the page.

Field types
Common Fields
- First Name, Last Name, Email, Phone
- Product — let the customer pick which of your products they bought. Show it as a Dropdown Menu or as Card Selection with product images.
- Channel Source — ask where the customer bought. It comes with Amazon, TikTok Shop, Walmart.com, and Direct, and you can add your own choices.
- Address — a full postal address with autocomplete.
- Yotpo Review — a shortcut that adds a complete review set in one click: which product, a star rating, a review title, and the review itself. Reviews collected this way can be sent to Yotpo from your flow.
More Fields
- Text, Long Text, Number, Date
- Dropdown / Radio, Checkbox, Rating, Country
- File Upload — customers can attach files. They appear under Audience → Form Uploads.
- Terms — a terms-and-conditions checkbox with your brand name filled in.
- Amazon Order ID
You can also add any custom field you've set up for your customers.
Field settings
Click a field to edit it in Field Settings:
- Label, Placeholder, and an optional Description shown under the field.
- Required — the form won't submit until it's filled in.
- Validation rules, like minimum length or rejecting numbers.
- Choices for dropdowns, checkboxes, and Channel Source.
- Countries — ask a field only of visitors in certain countries.
- Translations — add wording for other languages.
Colors and style
Form colors are set on the Form block itself, in the page editor's properties panel:
- Background Color and Text Color for the form.
- Field Background Color and Field Text Color for the input boxes.
- Container Style — Card or Flat.
- Button text and colors, including Submit Button Text, Button Color, and an optional gradient.
After someone submits
In Design, under After Submit, pick one:
The customer moves on to the page's success view. Design it with the Form Success block.
The form is swapped for a thank-you view right where it was, with no page change. Start from a layout (Simple thank-you or Image + button), then stack Image, Text, and Button pieces. Toggle After submit in the Live Preview to see exactly what customers will get.
If you leave it empty, customers see a plain "Thanks" message.
The Design panel also has One Question Per Screen, a Progress Indicator, and Animations.
What happens with the answers
- A customer record is created or updated, matched by email.
- Answers are saved to the customer's profile, so you can see them on the Customers page.
- Your flow carries on to its next steps — for example sending the customer to Klaviyo, Shopify, or Google Sheets.
